This Change Order modifies the applicable Borealis managed-governance agreement and order form solely for the customer and project identified below.

Customer legal name; underlying agreement date; order form reference; project or program reference; effective date of change.
Description of Change
Describe the requested change in service scope, governance cadence, deliverables, onboarding support, reviewer support, seat counts, workspace limits, AI usage limits, billing cadence, term, or other commercial terms. State expressly whether the change includes any new service commitment, response time, review turnaround, integration work, custom deliverable, or reliance language.
Commercial Impact
List any additional recurring fees, one-time fees, credits, revised setup fees, revised term commitments, revised invoicing schedule, and tax treatment.
No Implied Waivers or Commitments
Except as expressly modified in this Change Order, the underlying agreement, order form, service description, professional-boundaries exhibit, DPA, and incorporated policies remain unchanged and in full force. No change order may create legal-advice, compliance-guarantee, audit-guarantee, or unlimited-support obligations absent express executive and legal approval.
